Pulmonary immunothrombosis critical in #COVID19 #ARDS pathogenesis:
*local endotheliopathy due to specific deleterious effects of #SARSCoV2 infection upon pulmonary endothelial cells
* thrombocytopathy: multifactorial platelet activation
* enhanced vascular permeability, facilitating leucocyte extravasation/exacerbating alveolar infammation & edema
net effect? generation of platelet–fibrin rich thrombi disseminated throughout pulmonary vasculature.
Complex multifactorial mechanisms involved, unlikely one-size-fits-all approach will be enough: stratifying patients into subgroups, and tailored optimal treatment regimen
combination required.
